Please feel free to call our office at (724) 658-5144 to schedule an appointment. If you reach the answering machine, please leave your name, telephone number, and a brief message, and we will return your call as soon as possible.
After scheduling an appointment, you may want to download and complete the Patient-Dental-Registration-and-History.pdf, sign the Acknowledgment of Receipt of Notice of Privacy Practices, and read the Notice of Privacy Practices as well as Financial Policy. You may also complete these forms at the office on your first visit. Please have available an updated dental insurance card and information required for billing as outlined in our Office Financial Policy.

Insurance and Billing
We accept most traditional insurance plans as well as PPO networks. Dr. Mendel does not participate in Health Management Organizations. We accept checks, cash, Visa, Master Card and Discover. We also offer a flexible payment plan through the Medical Bureau of Pittsburgh. Please refer to our Financial Policy statement for additional details.
Payment Options
We accept checks, cash and credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details. We are happy to file insurance for your reimbursement as long as you are free to choose your own dentist.
